Research Participation Consent Form Department of Pharmacy Title of Project: Alcohol based hand scrubs with medicinal plant extracts Researcher(s): Student K.D.S.V.Karunanayaka and Dr. N.B.Parahitiyawa Your consent is being sought to participate in this study. Please read the following information carefully before you decide whether or not you consent to participate. Purpose of the research: The purpose of this study is to evaluate the efficacy of a novel alcohol based hand scrub product consists of the antimicrobial efficacy proven pure active ingredients of known medicinal plants. Procedure to be followed: You will be asked to keep your finger tip on portions of agar plate before, immediately and after 30 minutes of application of novel hand scrub on your finger tip . Finally you will also be asked to fill a simple questionnaire to evaluate your compliance. Discomforts/risks: The risks in this study are minimal. You may feel irritant or allergy condition. Safety measures will immediately be taken if you will be encountered with such a thing. Incentives/benefits for participation: You will be one of volunteer who give great support to evaluate the efficacy of a novel hand hygiene product which is essential for the prevention of Healthcare Associated Infections (HAI). The research itself will benefit to introduce an effective novel hand scrub to the society. Time duration of participation: Participation in the study will not exceed 45 minutes. Statement of confidentiality: Records will be kept confidential and will be available only to professional researchers and staff. If the results of this study are published, the data will be presented in group form and individual participants will not be identified. Voluntary participation: Your participation is voluntary. If you believe you have been in any way coerced into participation, please inform the researcher. Also, you may choose not to answer any question(s) that makes you uncomfortable. Termination of participation: You may choose to withdraw from the study at any time and still receive extra course credit.
