abstract the hiv/aids pandemic is one of the most serious health crises the world faces today. generally 34 million people were estimated living with hiv by the end of 2009 and 25 million people have died of this pandemic since 1981. however the discovery of antiretroviral therapy has slowed down the progression of hiv, reducing death rates. samuel b (2000). adherence is defined as a patient’s ability to follow a treatment plan take medication as prescribed. hiv/aids is a chronic and manageable disease and need adherence >95% reddy k.s. (20011). in clinic practice non-adherence is wide spread among patients with chronic disease like hiv/aids. challenges for both patients and health care providers estimate average rates of non-adherence to art range from 50% to 70%. william l.(2006). however, there are factors that hinder adherence to art which are: patient related factors, treatment regimen, diseases characteristics patient-provider relationship and clinical settings. if these factors are addressed the rate of non-adherence is reduce. measuring adherence is important because it is aimed at improving health and prevention of viral resistance. this can be done by self-pill count, pharmacy refill record, and electronic drug monitory. according to marshall becker in 1997, he assumed that persons will take a health related action if they feel that negative condition can be avoided. also that a person will avoid negative health condition, that is progressing to the stage of aids by taking his/her treatment as recommended. the research question was: what are the factors that influence clients’ adherence to antiretroviral therapy? the purpose of this study was to assess the factors that influence clients, adherence to art. in carrying out the study, the following guiding objectives were set: - to assess the clients perception and view about art - to determine the proportion of respondents who adhered to art. - to find out the reasons for non-adherence. - to recommend strategies that will help increase adherence to art the hypothesis was “it factors hindering adherence to art are identified and interventions made, clients will likely adhere to their treatment. the study was carried out in day hospital bamenda. it was a descriptive cross sectional design in which the investigator was a representative of the entire population living with hiv/aids and on antiretroviral therapy, age 15 years and above. a sample of 50 clients was recruited for this study. a structured questionnaire interview was used for data collection. data was analysed and presented in the form of tables, bar charts, pie charts, histograms and percentages. the result revealed that majority of the respondents had good knowledge about hiv/aids as 30(60) said there is no cure for hiv and a good number also said the most effective treatment to combat hiv/aids is arvs 44(88%). the study also revealed that side effects 19(30%) cause non-adherence and said the reasons they missed their medication is due to forgetfulness 13 (265), drugs not adherence 25 (50%) etc. finally the study revealed that there was a general positive attitude toward, the interpersonal relationships between care providers and respondents; 40(80%) which contribute to increase in adherence. however, there is still much to be done by the health providers as client’s needs continuing adherence counselling because art is a lifelong therapy. the factors affecting adherence were identified as: - non availability of drugs - forgetfulness - stigma etc. some recommendations were made to the following: to the day hospital, bamenda, brh.  adherence counselling should be reinforced and emphasis made on the importance of strict adherence.  client should be encouraged to disclose their status to at least one person in the family.  patient should be encouraged to have reminder aids like alarm clock to help them take their medication.  to the ministry of public health  art services should be made available in district and health centres to solve problem of transportation cost.  staff should be recruited to solve the problem of over work load and long waiting time in the clinic.  clients should be given two months refill of drugs to solve transportation cost.  they should ensure availability of arvs at all times
