mechanical valves: all patients with current-generation single tilting-disk or bileaflet valves should receive warfarin indefinitely to achieve a target inr of 2.5 (range 2.0–3.0). in patients at higher risk of thromboembolism (atrial fibrillation, mi, left atrial enlargement, endocardial damage, low ef, or history of systemic embolism), aspirin 75–100 mg/day should be added and the target inr raised to 3.0. heparin may be started around the fourth postoperative day if the inr is less than 1.8, always being cognizant of the potential for delayed tamponade in the anticoagulated postoperative patient. the patient can usually be discharged home once the inr is approaching the therapeutic range (generally > 1.8).
الصمامات الميكانيكية: يوصف الـ warfarin الدائم بعد زرع أيّ من الصمامات الميكانيكية الحديثة ذات الوريقة الوحيدة أو الوريقتين (مستوى الـ inr الهدف 2.5، والمدى 2.0-3.0)، ويُنصح بإضافة الـ aspirin بجرعة 75-100 ملغ/يوم وبرفع مستوى الـ inr الهدف إلى 3.0 في حال إرتفاع خطر حدوث الصمّات الخثرية (ضخامة الأذينة اليسرى، الرجفان الأذيني، إحتشاء العضلة القلبية، أذيّة شغاف القلب، ضعف الجزء المقذوف، أو وجود قصة سابقة لصمّة جهازية). يمكن أن نبدأ بتسريب الـ heparin حوالي اليوم الرابع بعد العمل الجراحي إذا لم يتجاوز مستوى الـ inr الـ 1.8، آخذين بعين الإعتبار إمكانية حدوث السطام التأموري المتأخر بعد العمل الجراحي عند المريض المُعالج بالمميعات.
