noch dahinter liegt die trachea, die sich auf höhe von bwk 4 in die beiden hauptbronchien teilt, die ihrerseits noch den herzbeutel tangieren.im oberen drittel des mediastinums ist der Ösophagus noch hinter der trachea anzutreffen, erst nach der bifurcatio tracheae legt er sich dem perikard an. der n. vagus legt sich auf der rechten seite recht schnell nach seinem eintritt in den thorax dem Ösophagus an, während er linke n. vagus zuerst noch den aortenbogen überqueren muss, um dann ebenfalls mit der speiseröhre nach kaudal und durch das diaphragma hindurch zu wandern.am weitesten dorsal, direkt vor der wirbelsäule, findet man weitere zirkulatorische strukturen: die v. azygos und hemiazygos, der ductus thoracicus und vor den rippenköpfchen der grenzstrang.aus dieser aufzählung ergibt sich folgendes: das mediastinum durchziehen venöse, arterielle, lymphatische, sympathische und parasympathische bedeutsame leitungsbahnen für die abdominellen organe. die gute funktion dieser organe ist abhängig von der struktur „mediastinum“.
