i, shannon iratumoana ruaihona raimona, was working march 26th, 2015. it was in te wera forest in the mighty taranaki that is where the bad tragedy happened it was very sad and this tragedy was was very unpreventable. it was a very cold and bitter morning at 7 a.m. when we set off to work. i saw james stewart oxenham loosening the strops off the high dump truck when a massive log weighing 800 kgs slipped free and fell very hard down on top of james. i called 111 and an ambulance arrived at a slightly warmer time around 8:15, i tried to resuscitate him but blood was running out of his mouth so i didn't try, but it was too late anyway james died from internal bleeding caused by the log landing on top of him and it looked to gruesome to even explain right now, but the results came back and he was pronounced dead. this was because the truck was quite overloaded so that the logs were over the side this is very dangerous. the sides of the bins did not support the heavy weight of the logs. but over all of this that has happening this man was a very good man and he could conquer the world with courage and his soft personality and i feel sorry to his beautiful wife and children and all of his work mate also outside of work. to stop this from happening in the future the dump truck should not be very overloaded and the sides of the truck should be made higher to get more logs in and so we can be safe around and there should have warning signs should be easy to see telling of the dangers of overloading. this is a truthful account of the events that happened,what caused it, and what steps can be taken to prevent this from happening in the future.
