(1) why this survey is conducted? most women get inserted jadelle implant as a contraceptive method for 5 years duration, but they attend to remove jadelle implant before complete 5 years due to varies side effects. this study help to identify what are the common side effects and what are any associated factors which help to recognize best suitable group to insert jadelle implant (2) can you say ‘no’ to participate for this survey after counselling? yes. you may withdraw your consent to participate for this survey at any time as your desire without mentioning reasons. (3) can you withdraw your consent from the survey after conformation of your consent to participate for the survey? your participation for this survey is voluntary. you will be free to cancel your conformation for participation to this survey at any time without giving reasons. in such occasions please be kind enough to notify that to the investigator as soon as possible.
