appendices appendix i information sheet __________________________________________________________________________________ unzabrec mukanyanga muzyamba ridgeway campus cuz school of medicine, munali campus p.o box 50110 p.o box 34625 lusaka, zambia lusaka, zambia tel 260-1-250753 tel: 293553/293698: mobile: 0968716817 email: unzarec@zamtel.zm email: amandamuzyamba@gmail.com “an analysis of the causes of waterborne diseases in lusaka. a case of kanyama township” introduction: the principal investigator of this study is mukanyanga muzyamba a student pursuing a master’s degree in public health at the cavendish university zambia. the main purpose of this study is to analyze the causes of waterborne disease and investigate their impact in kanyama township. the purpose of this study is to analyze the causes of waterborne diseases and to investigate their impact on kanyama township residents of lusaka, zambia, thereafter give recommendations on the way forward. the outcome will be cardinal as it will provide an in-depth understanding of causes of waterborne thereby providing suggestions/ recommendations on the remedy. procedure: the participants will be verbally asked if they will be willing to participate in the study. as a formal way of their acceptance to participate in the study, participants will be requested to sign a consent form. participants will be requested to fill in field questionnaires which will be self-administered. participants are free not to answer questions that may deem personal or otherwise. risks and / discomfort/ benefits: there are no foreseeable risks or discomforts to be associated with this research to participants. the benefits of the study to the participants will include first gaining a deeper understanding of the problem of attaining safe and clean water and high incidence of waterborne diseases and second the improvement of safe water supply quality through the recommendations that will be made to policy and decision makers and cooperating partners. confidentiality: participants will not be allowed to give their names, house numbers, or any information that would identify them. any information obtained during this study, which could identify them, would only be accessed by the principal researcher and not by any other person or reasons other than for this research. the responses to the questionnaires and interviews will not be shared with any other parties apart from the respondent and the researcher. freedom to withdraw: participation in this study will be voluntary and participants can therefore refuse to participate or withdraw at any time without harming their relationship with the researchers or cavendish university zambia, or in any other way receive a penalty or loss of benefits. participants have the right to seek further information from the researcher and/or research assistants.
